Episodic Visit: Gynecology Focused Note

Patient histories are a building block of the diagnosis and treatment. By effectively interviewing patients in their care, advanced practice nurses can piece together facts to construct a relevant history that can lead to assessment and treatment.

For this Focused Note Assignment, you will select a patient with gynecologic conditions from your clinical experience and construct a patient history, assess and diagnose the patient’s health condition(s), and justify the best treatment option(s) for the patient.

Advanced nurse practitioners can play an active role in these important visits. This role can include a physical examination as well as collection of details about such factors as nutrition habits, sexual activity, stress, and more. By participating in comprehensive well-woman exams, advanced nurse practitioners can help patients engage in preventative health.

For this Assignment, you will complete your well-woman exam using a focused note format in which you will gather patient information, relevant diagnostic and treatment information and reflect on health promotion and disease prevention in light of patient factors, such as age, ethnic group, past medical history (PMH), socio-economic status, cultural background, etc.

Current health status, including chief concern or complaint of the patient

Contraception method (if any)

Patient history, including medical history, family medical history, gynecologic history, obstetric history, and personal social history (as appropriate to current problem)

Labs, tests, and other diagnostics

Differential diagnoses

Management plan, including diagnosis, treatment, patient education, and follow-up care

Week 10 Assignment 2

Final Clinical Evaluation

Hopefully, at this point in the practicum, your confidence level in performing procedures and demonstrating clinical skills has risen. In preparation for your Final Evaluation, you may wish to review your printed-out Clinical Skills List and revisit the goals and objectives you established in Weeks 1 and 2. Remember that as an advanced practice nurse, you welcome evaluation and supportive feedback as growth opportunities.

For this Assignment, your Preceptor will assess your clinical skills in a Final Evaluation.

Following your Final Evaluation, your Clinical Faculty and Preceptor will meet privately with you to discuss your results and feedback. Once you receive your feedback, compare your Preceptor’s evaluation with your self-evaluation to better understand your clinical performance.

Clinical Evaluation Grades

Your clinical instructor will complete an evaluation with feedback from your preceptor and evaluation of your work.    Meditrek calculates an average score which will be used to determine the score that will be entered into Blackboard for your evaluation grade. 

Students must demonstrate achievement of the clinical learning outcomes for the course by achieving a minimum score of 3 (meets requirements) on all criteria on the final evaluation. Scores of less than 3 on any criteria can lead to course failure or the implementation of a clinical performance improvement plan.
